Visiting Clergy Forms

Clergy and religious planning to visit this Archdiocese for Ministry or Apostolate purposes must notify the Vicar General’s office about their upcoming visit and complete the following forms below:

• Faculty Request and Grant Form for visiting clergy: and
• Safeguarding Statement Declaration for visiting clergy.

Clergy from the Archdiocese of Hobart who are visiting other states should use the form below:

• Safeguarding Statement Declaration for AoH clergy visiting other states.
